Preparing for the initial consultation

Before your appointment

Initial Clinic appointment

Please bring along:

1. Your Medicare card (and DVA gold card if applicable to you)

2. Referral letter

  • We prefer electronic referrals forwarded before your appointment by fax or email. If you can’t then please bring a hard copy to your appointment.
  • A valid referral letter is required for Medicare rebate. 
  • Without a referral you will be out of pocket for the whole fee without any Medicare rebate.

3. Medications 

  • Bring a list of your medications, actual bottles/patches/inhalers or your blister pack.
  • This includes as required medications or over the counter and naturopathic treatments.

4. Test results

  • Bring any relevant blood tests, imaging reports and scans

5. Family member or friend

  • We strongly would recommend coming to our appointment with a family member. Our consultations might touch on sensitive topics and it can be useful to have a support person to participate in our consultations. 

Getting here:

At your appointment

  • Please aim to arrive at least ten minutes early to your appointment.
  • At Concord Specialist Centre – head through to the reception where our staff will be ready to check you in.
  • Your first consultation takes at least 60 minutes depending on the nature and complexity of your problem/s.
  • Fees are payable by credit or debit card at the time of your appointment.
  • We do not accept cash or cheques. 

After your appointment

1. Be sure to write down as much as you can immediately after your appointment to help with recollection afterwards.

2. You might think of questions after leaving the specialist – please write them all down and bring them to your next consultation and we will be happy to work through them with you. Alternatively you may be able to discuss with your general practitioner in the meantime.

3. Please take care of your forms and prescriptions provided to you during the consultation.

for Home visits

1. We shall contact you to discuss regarding details of our home visit. (Please note criteria apply for our home visit services)

2. Please obtain a referral letter from your doctor. We prefer electronic referrals forwarded before the home visit by fax or email. If you can’t then please retain a hard copy yourself.

3. We would recommend having a family member there to accompany you during the visit. 

4. Please arrange and show us your medications and relevant investigation results (including blood tests and scans)

5. The aim of home visit is to assess you in your own environment. We would recommend wearing comfortable clothes, footwears and having your walking aid available. 

Residential Aged Care Facility Visits

1. Please obtain a referral letter from your doctor

2. We shall contact the facility manager and you to arrange for a time of review at the facility.

3. We would recommend having a family member there during our review.

4. Kindly ready the following in preparation for your appointment:

  • Identification (e.g. drivers license, Medicare card)
  • Any recent investigation results (e.g. recent blood test results, recent ECG reports, CT scan results, MRI scan results)
  • Any recent correspondences from other doctors

Telehealth Medicine

1. Please obtain a referral letter from your doctor

2. We shall contact you to arrange for a time of review and provide your with further instructions on how to access telemedicine.

3. We would recommend having a family member there during our review.

4. Kindly ready the following in preparation for your appointment:

  • Identification (e.g. drivers license, Medicare card)
  • Any recent investigation results (e.g. recent blood test results, recent ECG reports, CT scan results, MRI scan results)
  • This can be emailed or fax to us prior to the review
